Instructions

Collect all your personal information together (address, phone number, $20 registration fee and your parent or guardian's permission if required) and have it ready to enter into the form.

Please read the questions first and think about your answers to some of the Questionaire-style questions further down the application. Also make sure you choose a nick-name that is both important to you and defines who you are as a person.

Please note that applicants who choose to use inappropriate or vulgar nicknames will be automatically disqualified from the competition.

Also make sure that you have a head-shot photograph available for upload. This must have your head and shoulders fully in the frame of the photo as per the example below.

Photos uploaded should be no larger than 2mb in size and be large enough to view onscreen. A recommended minimum size would be 300 x 300 pixels.
